Client Forms

Below are my client forms to help us start on the same page and to ensure we are both aware of our rights and responsibilities in a therapeutic relationship. I have an online Client Portal through which you can receive, read, and digitally sign these documents, so unless you prefer hard copies, there is no need to print and sign these. However, if you like hard copies, discuss this with me, and we will arrange delivery, review, and signatures before our intake appointment.
